Targeted Care Package Key Worker
- Full time or Part time
- Fixed contract with end date 30/06/2027
Targeted Care Packages (TCPs) provide creative and individualised placement and support options for children and young people with complex needs and behaviours.
In this role, you will provide intensive case management and placement support functions in line with the case plan for children and young people subject to a (TCP). You will assist children and young people to achieve positive outcomes by maintaining a creative, flexible, optimistic, and solution-focused approach to your work.
To be considered you must possess the following:
- A relevant tertiary qualification in Social Work, Psychology, or equivalent at degree level with substantial experience or a relevant Diploma with substantial experience in the relevant service stream
- Certificate IV in Child, Youth, and Family Intervention including the completion of the residential care mandatory 4 top up units
- Demonstrated experience working with Child Protection
- Experience working with CRIS and CRISP
- Relevant case management experience.
- Client service focused: committed to providing exceptional customer service across all channels written, phone and face to face
- Proof of Right to Work in Australia
- Current Employment Working with Children Check
- Current Drivers Licence
- Own reliable vehicle with Comprehensive Car Insurance
- Satisfactory National Policy Check & International Police Check if required
- NDIS Worker Screening Check
- Completion of the NDIS Worker Orientation Module
- First Aid Certificate and current CPR
