About the Role

The Disability Community Program Coordinator (Outreach & Home-Based Disability Services) is responsible for providing operational services in the delivery of outreach and home-based disability services.

This role contributes to the Disability Team, ensuring high-quality service delivery, supervising staff, and ensuring staff compliance and quality client care. A key component of this role is to support the Disability Community Program Manager with day-to-day tasks.

Key responsibilities include:

Under the direction of the Disability Community Program Manager, this role will:

  • Coordinate the day-to-day operations of community and home-based disability programs.
  • Provide supervision of Support Workers.
  • Coordinating and delivering internal training
  • Actively ensuring Support Worker compliance
  • Assisting in recruitment of new Support Workers
  • Processing timesheets and leave requests.
  • Provide a point of contact for new client referrals and assist in manage client cases effectively.
  • Maintain timely, accurate documentation and communication.
  • Attend client and care team meetings to support effective case management.
  • Ensure service delivery in out-of-home care programs is child-focused, trauma-informed, and aligned
  • with best practice standards.
  • Provide operational leadership across disability programs, ensuring compliance with legislation,
  • regulatory frameworks, and quality standards.
  • Collaborate with finance and human resources to ensure effective resource management within
  • budget constraints.
  • Build and maintain strong relationships with key internal and external stakeholders, funders, and other
  • Managing internal and external reporting requirements are met.
  • Drive continuous improvement in service delivery

We’re looking for someone who has:

  • Extensive experience in NDIS service delivery and disability support programs.
  • Proven experience at a coordinator level, with the ability to support teams using a trauma-informed
  • Strong understanding of government policies, regulations, and processes related to NDIS services.
  • Up-to-date knowledge of current trends and challenges in NDIS service delivery.
  • High level of cultural competency, with the ability to provide culturally safe and inclusive services to
  • Aboriginal and Torres Strait Islander communities, ensuring connections to country, culture, and
  • Strong stakeholder engagement and relationship management skills.
  • Experience working with children and young people in a relevant context.

To be successful, you’ll need:

  • Completed a Cert IV in Disability or similar qualification.
  • Current Employment Working with Children Check.
  • Current Victorian Drivers Licence.
  • Satisfactory National Police Check & International Police Check if required.
  • NDIS Worker Screening Check.
  • Completion of the NDIS Worker Orientation Module.
  • First Aid and CPR Certificate (or willing to obtain).
